Differentiating between a medial meniscus tear and pes anserine bursitis is critical, as they occur in the exact same anatomical region but require different mechanical interventions. Symptoms of a meniscus tear include deep joint locking and joint-line pain, while bursitis presents as superficial, burning tenderness below the joint. Physiotherapy assists in managing both conditions by using targeted clinical testing to identify the failing structure, helping to optimize movement and safely restore joint capacity. The biomechanical root cause dictates the treatment: managing internal rotational shear or stopping external tendon friction.

Your mind jumps straight to a meniscus tear and the terrifying prospect of orthopedic surgery. You ice the knee, wrap it in a heavy tensor bandage, and avoid stairs entirely. But what if the pain isn't actually coming from the cartilage deep inside the joint hinge?
Differentiating between a meniscus cartilage tear and pes anserine bursitis is one of the most common diagnostic puzzles we solve at Rehab Mechanics. Because both conditions cause pain on the inside of the leg, they are constantly misdiagnosed by walk-in clinics. While a definitive diagnosis requires a clinical evaluation, you can use a few simple mechanical clues to better understand what is happening under the skin and start guiding your recovery safely.
Structural / Biomechanical Analysis
To self-assess accurately, we must perform a biomechanical analysis of the distinct structures residing on the inside of the knee.
The Medial Meniscus (Intra-Articular)
The medial meniscus is a C-shaped piece of tough cartilage that sits deep inside the watertight capsule of the knee joint.
The Joint Compression Pattern: It acts as a shock absorber between the thigh bone and shin bone. It is injured by extreme, twisting shear forces (like pivoting on a planted foot) or natural, age-related degeneration. When it tears, the pain and swelling are trapped deep inside the joint line.
The Pes Anserine Bursa (Extra-Articular)
The pes anserine bursa is a fluid-filled sac that sits completely outside the joint, roughly 2 to 3 inches below the actual knee hinge.
The Tendon Failure: Three thick tendons (the hamstrings and adductors) anchor directly over this sac. When the knee caves inward (valgus collapse), these tendons whip violently over the bone, crushing the bursa sac. This causes extreme, friction-based inflammation that has absolutely nothing to do with the cartilage inside the knee.
Clinical Red Flags (The 4-Point Checklist)
We meticulously differentiate the source of medial knee pain using specific mechanical loading tests. Here are four clinical clues you can look for:
The Touch Test (Location): True medial meniscus pain tracks directly along the horizontal joint line where the bones meet. Pressing firmly 2 to 3 inches lower, on the flat, upper inside part of your shin bone, will cause sharp, localized tenderness if the superficial bursa is inflamed.
Mechanical Catching vs. Clear Hinging: Does your knee physically lock, click, or feel like it catches when you try to straighten it? That points to a structural meniscus flap getting caught in the joint. If your knee bends and straightens smoothly but simply stings under load, the superficial bursa is the likely target.
The Stair Descent Trigger: Walking down stairs places massive eccentric load on the hamstring tendons. If descending stairs triggers a sharp, burning pain on the inner shin, but twisting the knee on flat ground does not, it points heavily to bursitis.
The Swelling Profile: A meniscus tear causes the entire joint capsule to puff up like a water balloon (effusion), giving it a "boggy" end feel. Bursitis causes a distinct, localized pocket of swelling directly over the shin bone, leaving the rest of the knee relatively normal.

The Rehab Mechanics Corrective Protocol
