No. The grinding or "sandpaper" sound in your neck (crepitus) is usually caused by stiff facet joints and gas bubbles shifting in the joint fluid, not bone-on-bone arthritis. When these patients attempt to roll their head in a circle or turn their neck to check a blind spot while driving, they hear a loud, gritty, grinding, or "sandpaper-like" noise originating deep inside their spine. Sometimes it sounds like Rice Krispies popping right at the base of the skull.

The medical term for this sound is crepitus. For a patient, hearing their own spine grind is incredibly frightening. The immediate, terrifying assumption is that their neck cartilage has completely disintegrated, that they are grinding raw bone-on-bone, and that they are suffering from severe, irreversible Cervical Osteoarthritis.

Your neck consists of seven vertebrae (C1 to C7). At the back of each vertebra, there are tiny, overlapping hinges called the facet joints.

3. Facet Joint Stiffness (The "Sandpaper" Grind)

This requires immediate clinical intervention.
